This form exists as a way for paramedics to report patient or paramedic events that may lead to, a worsened patient outcome or harm the paramedic either physically or psychologically.
These reports may be related to systems, operations, government legislation, cultural pressures, drug administration or any clinical aspect of patient care. These events also include patient near misses (i.e. close calls) that are recognized before they actually occur. This form DOES NOT replace standard workplace reporting systems, but instead serves as an avenue for practitioners to inform the Association of frontline challenges.
Please note, this portal is not monitored day-to-day. The reports made are compiled and used to inform work and lobbying done by the association to affect change and individual reports are not acted on or replied to. Thank you for your report, you are making a difference.
