This form exists as a way for paramedics to report patient or paramedic events that may lead to, or has the potential to lead to, a worsened patient outcome or harm, either physically or psychologically, to the paramedic. These may be related to systems, operations, government legislation, cultural pressures, drug administration or any clinical aspect of patient care. These events also include patient near misses (i.e. close calls) that are recognized before they actually occur. This form DOES NOT replace standard workplace reporting systems, but instead serves as an avenue for practitioners to inform the Association of frontline challenges.

This form system is strictly anonymous. Individuals who submit information to this website cannot be identified. Individuals must not submit information that identifies any patient, EMS practitioner, EMS service, air medical service, date of incident, specific location of incident, or any other information that may identify any of these entities. Reports with these identifiers will be deleted from the system and/or may be subject to HPA mandatory reporting requirements.

Reports submitted using this form will be used for statistical data to improve our profession. There will be no action taken as a direct result of any single report, however multiple reports with commonalities will help  inform the association in influencing change for our profession.
